How Fidelis Care Can Help

Case Management

Fidelis Care offers case management services to members by phone. Our case managers can provide support and encouragement to help improve your health. We offer programs staffed by registered nurses and other trained professionals for the following conditions:

  • Asthma 
  • Diabetes 
  • Heart Disease
  • Prenatal (Maternal Health)
  • Behavioral Health 

Fidelis Care also offers Complex Case Management.

Population Health Management

Fidelis Care’s Population Health Management (PHM) programs support the needs of Fidelis Care members throughout the health care journey.

Fidelis Care’s PHM programs are fully voluntary, and offer a range of services tailored to each member who wishes to opt in, including:

  • Flu shot reminders
  • Hemophilia and sickle cell disease support
  • Successful transition from hospital to home
  • Health Homes referrals
  • Support for members with both behavioral and medical needs
  • Medication reconciliation
  • Health education

Fidelis Care members are notified each year about all available PHM programs by mail, and online at Members who do not require PHM programs may still receive support through telephone outreach, Fidelis Care staff, monitoring for gaps in care, and follow-up calls with providers.
