Health Risk Assessment Incentive Program

For members in Wellcare Fidelis Dual Access (HMO D-SNP) Plan 001

Wellcare Fidelis Dual Access members may be eligible to receive a $10 incentive (gift card) for completing their Health Risk Assessment (HRA). Members must complete their HRA by  December 31, 2023, to be eligible. You can claim your reward until March 31, 2024, if you have completed your HRA before the end of 2023.  If you still need to complete your annual Health Risk Assessment, call our Fidelis Care HRA team at  1-800-247-1441, ext. 16518. We are open 8:30 AM – 5 PM Monday through Friday.


Complete Your HRA Today

If you are a Wellcare Fidelis Dual Access member and you have already completed your HRA, you also may be eligible for the gift card.

Accessing Your Reward

Once you have completed your HRA and allowed 30 days for processing, you will receive an email from our partner, Healthmine. The two ways to access your reward are through the Healthmine website or telephone number below:

  1. Access the link within the email to go directly to the Healthmine page: https://wellcare.healthmine.com.
  2. Call My Wellcare Rewards: 1-866-550-1590. 9 AM to 9 PM

You can request either an electronic or physical gift card.

Physical Cards: Healthmine will mail physical gift cards to eligible members within seven business days of redemption/selection of the gift card. Cards are non-refundable and cannot be exchanged. Gift card fulfillment will be within 23 calendar days. Note: If you do not receive your card within two weeks of placing your order, please call 1-866-550-1590. 9 AM to 9 PM. Verification will be made that the original card was not used.

Digital Cards: Healthmine will email electronic gift cards to eligible members within three business days of redemption. Gift card fulfillment will be within 23 calendar days. These cards are non-refundable and cannot be exchanged. Please make sure to check your spam folder.

Note: Healthmine is an independent vendor with no affiliation to our previous program and cannot honor anything outside of their program parameters.

 

Additional Information

Wellcare is the Medicare brand for Centene Corporation, an HMO, PPO, PFFS, PDP plan with a Medicare contract and is an approved Part D Sponsor. Our D-SNP plans have a contract with the state Medicaid program. Enrollment in our plans depends on contract renewal.

From October 1 to March 31, you can call us 7 days a week from 8 a.m. to 8 p.m. From April 1 to September 30, you can call us Monday through Friday from 8 a.m. to 8 p.m. A messaging system is used after hours, weekends and on federal holidays.
