Thank you for your interest in joining the Fidelis Care Provider Network! Fidelis Care participating providers embody our mission to provide quality, affordable health care with dignity and respect, to people of all ages and at all stages of life.

We currently accept Primary Care and Specialist Physician requests online. We will do our best to respond to all inquiries as quickly as possible.

Fidelis Care expects all Primary Care Providers (PCPs) to abide by our Access and Availability Standards.
